TIMELY: A patient-centred lifestyle program to support the continuum of care in patients with coronary artery disease using eHealth and artificial intelligence

The TIMELY platform and application (‘app’) have been developed to maintain a healthy lifestyle after cardiac rehabilitation for patients with coronary artery disease. Behavioural change models and techniques will be employed to empower and motivate patients to adopt a healthy lifestyle over a sustained period of time. TIMELY will become the first Artificial Intelligence-powered, patient-centred eHealth program that adapts and customizes CR to meet the needs of patients. Patients will be assisted in changing their lifestyle and managing their illness with the goal to reduce the progression of CVD.
